Aftercare

Aftercare – very much in the sense of coaching – is an accompanied offer on an hourly basis. The aftercare service is aimed at people who still need situational support after leaving an Erlenhof | Stiftung service and who already live in their own apartments. Socio-educationally trained residential coaches work together with the person concerned on the clearly defined objectives set out in advance at fixed appointments. The service can also be set up as support for reintegration into the family of origin (similar to socio-educational family support) and is financed by the canton if the person has previously been placed in a child and youth welfare institution in the Canton of Basel-Landschaft.

People aged 18 and over who live fully independently in their own home and manage their everyday life (living and working) independently can receive hourly support from qualified socio-educational staff if indicated.

The “Aftercare” service provides sequential support to the person concerned in specific everyday issues within a coaching process, which are agreed in advance on the basis of objectives. The person receives essential help in securing their daily work structure, dealing with administrative tasks, everyday life and integration within the social environment. Additional tasks of the socio-pedagogical aftercare are the assignment of a specialist who accompanies and coordinates crisis situations if necessary.

The aftercare service is also aimed at family systems (especially in the case of repatriation) whose children come from long-term residential care within the Canton of Basel-Landschaft. If indicated, the aftercare service is based on the concept of socio-educational family support.

The service is provided on an hourly basis and is needs-oriented.
